What is the 17 OHP test?

17-OHP or 17 Hydroxy progesterone is a blood test measuring the amount of 17-OH progesterone, a hormone produced by the adrenal glands and sex glands. Adrenal glands are present on top of the kidneys and produce another important hormone called cortisol. 17 OHP is converted to cortisol with the help of certain enzymes.

Some people, however, cannot make enough cortisol because they lack an enzyme (21-hydroxylase) in the adrenal glands that are needed to make it. They’ll have a buildup of 17-hydroxyprogesterone in the blood because it’s not converted to cortisol. This test is primarily performed to check infants for an inherited disorder that affects the adrenal gland called congenital adrenal hyperplasia (CAH). 17-OH progesterone Test Price usually ranges from Rs 750 to Rs 1900, and in-home sample collection may also be availed.

What is the purpose of the 17 OHP Test?

17 OHP Test is usually ordered by a doctor for-

  • Screening newborn babies for Congenital Adrenal Hyperplasia (CAH) in infants.
  • To check for Acquired Congenital adrenal hyperplasia in children or young adults showing certain signs and symptoms.
  • To evaluate females with hirsutism or infertility, both can result from adult-onset CAH.
  • Monitoring of treatment in patients of CAH.
17-OHP Test Summary

What is CAH?

Congenital Adrenal Hyperplasia (CAH) is a genetic condition due to a deficiency of certain enzymes.
It is of 2 types:

  • Classic
  • Non-classic

Classic CAH is detected in infancy, whereas Non-classic CAH is milder and involves children & young adults. 17-OHP is elevated in all these patients with CAH.

The signs and symptoms of CAH in infants include:

  • genitals that are ambiguous, which means not clearly male or female
  • pubic hair
  • acne
  • a lack of energy
  • disinterest in eating
  • dehydration
  • low blood pressure
  • vomiting

Some symptoms of acquired CAH, in a girl or young woman may be:

  • Excess hair growth in places where adult men grow hair
  • Deep voice
  • an increase in muscle mass
  • Absence of menses
  • Infertility

Some symptoms of acquired CAH, in a young man may be:

  • Deep voice
  • Early onset of puberty (precocious puberty)
  • Severe acne
  • Well defined muscles
  • Enlarged penis
  • Infertility

Preparation Before the Test

An early morning specimen is preferred in the follicular phase of the cycle (first 5 days following menstruation) and off the pill.

For infants, samples should be taken at least 48 hours after birth, or levels will be falsely raised.

Do inform your doctor if you are taking any medicine, both prescription and non-prescription, ayurvedic, etc. Your doctor may advise you to stop the ones that can affect the test results, like corticosteroids. DO NOT stop any medication on your own.

17-OHP Test Procedure

It is a fairly simple diagnostic blood test. A blood sample is taken from the vein in your arm or hand by the following steps-

  1. A technician (phlebotomist) may tie a tourniquet (elastic band) to your arm.
  2. The phlebotomist will locate your blood vessel.
  3. The site is cleaned with spirit.
  4. A Needle is inserted into your vein to draw the blood.
  5. A blood sample is then withdrawn into a syringe or a vial.
  6. Cotton is placed on the puncture site with mild pressure till the bleeding stops.
  7. A small band-aid is then placed which may be removed after an hour.

The entire procedure takes only a few minutes.

Risks During the Test

It is a very simple and safe test. The risks involved are the same as that in any other blood test. These include-

  • Infection at the puncture site if proper sterilization protocol is not followed and the needle is contaminated. This is, however, very rare. You may ensure that your phlebotomist takes out the needle in front of you from a sterile pack.
  • Multiple pricks if the blood vessel is not easily found
  • Excessive bleeding at the puncture site- Please inform your healthcare provider if you are taking any blood thinners or have any bleeding disorders.
  • Light-headedness due to stress or heavy bleeding.

Minor pain and bruising that might occur will resolve on their own in a very short time.

17-OHP Test Results and Interpretation

A higher amount of 17 OH progesterone indicates a lower production of cortisol and vice versa.

Normal Levels

The normal ranges of 17 OHP vary according to age and gender. The reference ranges with respect to age and gender are elaborated in the table given below:

GenderAgeRange of 17 OHP in ng/mL
AnyFoetus (umbilical cord blood)10-30
Newborns<6.3
Infants<1
FemalesPrepubertal<1.0
Follicular Phase of Menstrual Phase0.1-0.8
Luteal Phase of Menstrual Phase0.6-2.3
Ovulatory Phase of Menstrual Phase0.3-1.4
Pregnancy2.0-12
Post Menopause0.13-0.51
MalesPrepubertal<1.1
Adult<2.2

How to read 17 OHP Test Results?

17 OHP Levels in ng/mLRemarks
<2Diagnosis of CAH is unlikely, provided the sample is given in the morning (8-10 AM) during the follicular phase in menstruating women
2-10Indeterminate, ACTH Stimulation test recommended to differentiate between PCOS and Non-classical CAH
>10Highly suggestive of CAH

Higher Than Normal Levels

A high level of 17-OH progesterone may be due to:

  • Tumors of the adrenal gland
  • Congenital adrenal hyperplasia (CAH)

Lower Than Normal Levels

Low levels of 17 OH progesterone may be found in adrenal hypofunction.

Adrenal hypofunction can be of either adrenal origin or hypothalamic-pituitary origin (due to suppression of HPA axis most likely due to therapeutic administration of glucocorticosteroids for a long time).

Limitations of the 17 OHP Test

All new CAH cases should be confirmed with a urine steroid profile. The concentration of 17-OHP in newborns varies with age, weight, prematurity, and twinning. Premature, sick, or stressed infants have higher 17-OHP values leading to false positive screen results. Antenatal corticosteroid treatment may reduce 17-OHP levels resulting in a false negative screen.

Conclusion

A doctor will usually order a 17-OH progesterone test for a young child, teenager, or adult who displays some of the classic symptoms of CAH. The sooner a person with CAH is diagnosed and treated, the less likely they are to experience complications.

Regardless of the results, it is essential for you to discuss them with your healthcare provider for a better understanding and course of action.

Exercise, meditate, and sleep well! We wish you a happy, healthy, and relaxed life!

FAQs

Who can get CAH?

CAH is a genetic disorder in which both parents must have the genetic mutation that causes CAH. A parent may be a carrier of the gene, which means they have the gene but usually don’t have symptoms of the disease. If both parents are carriers, each child has a 25% chance of having the condition.

Is 17 OH Progesterone Elevated in PCOS?

The levels of 17 OH Progesterone may be elevated in PCOS. However, they are NOT significantly elevated.

What does it mean when the 17 hydroxyprogesterone level is low?

Low 17 OHP levels indicate adrenal hypofunction of either adrenal or hypothalamic/ pituitary origin.
